We cater services to keep them as independent as possible by meeting in their home to assess their functional, cognitive, emotional and social needs. The assessment is design to get a comprehensive picture of the member’s needs and desires. Once the assessment is complete, creating an individual plan of care to meet the member’s needs is the next step.
